Premature ejaculation is one of the most common male health problems, and affects roughly 20-30% of all men. It is very common, and causes the man to ejaculate sooner than he or his partner desire during intercourse.
Premature ejaculation can cause high levels of anxiety, depression and even lead to erectile dysfunction or a desire to completely avoid intercourse. To prevent mental deterioration, and re-establish sexual health, it is recommended that you get examination and treatment as soon as possible.
CAUSES
The causes for premature ejaculation might be both emotional or physical.
– Mental factors include: anxiety, poor self-confidence, relationship issues, depression, anxiety, guilt.
– Physical issues might also be the cause: unbalanced hormone levels, inflammation or infection of the prostate or urethra, unbalanced brain chemicals (neurotransmitters)
EXAMINATION
Discussing with your doctor how early the ejaculation occurs, when the problem started, and the state of your mental health and sex life, will help him determine what type of treatment will be needed.
Practitioners will also ask for a physical exam to be performed
TREATMENT
Premature ejaculation can be treated by psychological/ behavioral therapy, or medical therapy
– the stop and start technique, where after stimulation, you stop arousal for 30 seconds (or until it goes away), and then start again, repeating until ejaculation
– the ‘’Squeeze’’ technique, which is similar, but requiring to squeeze the tip of the penis in order to stop the erection
– pelvic stimulation, like performing Kegel exercises might strengthen pelvic muscles and help prevent Premature ejaculation
– psychological counseling with a professional can help work through mental issues that may be contributing to the patient’s premature ejaculation
– oral medication, like tramadol, paroxetine, fluoxetine, sertraline and clomipramine that affect a man’s serotonin levels and can help delay ejaculation
– anesthetic creams, like prilocaine or lidocaine, that make the penis less sensitive, thus delaying ejaculation. These are more effective when combined with the use of a thick condom
– Intercavernosal injections , which are also used in cases of erectile dysfunction, can be used to treat premature ejaculation, as they facilitate blood flow and help to gain better control of erections and better maintain it.
